Association between Oral Fluoroquinolone Use and Retinal Detachment: A Retrospective Cohort Study in Korea

Abstract
Objective: A recent study of ophthalmologic patients found a strong association between fluoroquinolone use and retinal
detachment. Given the prevalent use of fluoroquinolones, investigation of this association is necessary in the Korean
population. This study aimed to investigate whether oral fluoroquinolone use is associated with an increased risk of retinal
detachment.

Methods: A nationwide, population-based cohort study was conducted using the Korea national health insurance claims database from 2015 through 2016. The cohort included 4,201,876 episodes of fluoroquinolone use and 10,852,399 episodes of amoxicillin use. Cox¡¯s proportional hazard model was used to estimate hazard ratios (HRs) for incident retinal detachment, adjusting for a propensity score.

Results: A total of 4,807 cases of retinal detachment occurred; 1,309 in fluoroquinolone users and 3,498 in amoxicillin users. The crude incidence rate was 6.3 cases per 10,000 person-years in fluoroquinolone users compared with 6.5 in amoxicillin users. Compared with amoxicillin use, fluoroquinolone use was not associated with a significantly increased risk of retinal detachment: the adjusted HRs was 0.96 (95%CI, 0.90-1.04). Use of fluoroquinolones was not associated with an increased risk of rhegmatogenous retinal detachment, exudative retinal detachment and central serous chorioretinopathy, with adjusted hazard ratios of 0.98 (95%CI, 0.75-1.19), 0.98 (95%CI, 0.85-1.12) and 0.95 (95%CI, 0.87-1.04), respectively.

Conclusion: In this cohort study based on the Korean population, oral fluoroquinolone use was not associated with increased risk of retinal detachment.
